Summary;

Responsible for the coordination of the MDS process, based on the comprehensive assessments of the resident's needs.

Essential Duties and Responsibilities include the following. Other duties may be assigned.

• Completes initial assessment of residents and all subsequent assessments.
• Updates physician's orders as needed.
• Responsible for entire MDS process.
• Reviews MDS assessments when there is a change in a resident's condition and/or quarterly.
• Develops Comprehensive Care Plan with the Interdisciplinary Team.
• Makes recommendations for charting requirements.
• Informs staff of any changes regarding resident care and care issues.
• Monitors "RUG" scores for optimal reimbursement.
• May be required to take on-call needs and assist with routine LPN duties.

*Maintain the confidentiality of all resident, staff and organization information.

Education and/or Experience

• Must have a minimum of two years Long Term Care experience.

Certificates, Licenses, Registrations

• Must be a Registered Nurse in the State of Ohio.
