Position Type: Per Diem, Home Health PT – Part A and Part B services

Job Description:
We are looking for a dedicated Physical Therapist to serve patients in the Rockland area. The role requires a minimum commitment of 15 visits per week. This per diem position involves providing high-quality care to patients in their homes, helping them achieve their physical therapy goals.

Key Responsibilities:

• Conduct thorough evaluations of patients' physical conditions and develop individualized treatment plans.
• Implement therapy interventions aimed at improving mobility, strength, and overall function.
• Collaborate with interdisciplinary teams to provide holistic care.
• Educate patients and caregivers on exercises, home safety, and self-care strategies.
• Maintain detailed and accurate patient records as per documentation requirements.

Compensation:

• $80 per visit
