Job Description

Welcome House is seeking a compassionate, motivated HUD Service Coordinator to support individuals and families experiencing homelessness in achieving stable income and housing. This professional, direct-service role provides ongoing case management, assessment, and connection to community resources while upholding Welcome House’s Core Values: Compassion, Advocacy, Integrity, Quality, Leadership, and Community.

Key Responsibilities:

• Manage a caseload of 35–45 participants, meeting regularly to review budgets, housing stability, and case plans.
• Conduct bi-monthly home visits and ensure all HUD compliance requirements are met.
• Provide outreach to individuals experiencing homelessness, including those in shelters, on the streets, and in rural Northern Kentucky.
• Coordinate access to services, advocate for participants, and support client self-determination.
• Maintain accurate documentation, including case notes, status changes, and HMIS (Vesta) entries.
• Collaborate with internal Homeless Solutions, Housing, and Shelter teams; represent the agency at community meetings.
• Assist with data reporting, analysis, and administrative duties as needed.

Qualifications:

• Bachelor’s degree or equivalent preferred.
• 1–3 years of experience in social services.
• Experience working with multicultural populations.
• Strong problem-solving, communication, documentation, and conflict management skills.
• Ability to work independently and collaboratively; strong knowledge of community resources.

Key Competencies:

• Effective client engagement and understanding of poverty.
• Knowledge of mental health, substance use, and job-readiness resources.
• Leadership participation within the case management team.
• Meets compliance and reporting expectations with minimal supervision.

Working Conditions:

• Office-based work across a three-floor building (stairs only).
• Travel required 35–45% of the time.
• Frequent computer use and desk work; must be able to lift 50+ lbs and move throughout all floors.
