Job Description

Location: 65 Memorial Rd WH BBSQ (10059) Shift: Monday-Friday, 8a-4:30p Join a leading healthcare network where every moment matters, serving a diverse patient population across Connecticut and Rhode Island. This role involves coordinating patient care throughout the continuum, acting as a patient advocate, educator, and liaison. Responsibilities include navigating patients through pre-procedure testing, providing education on VT procedures, and ensuring timely service delivery. The position requires independent assessment, collaboration with healthcare teams, and maintaining a national registry database. ~ Qualifications include a BSN, 5 years nursing experience, CT RN or PA/APRN license, and 2 years of supervisory/education experience. Travel is required.
