The Department of Social Services strives to make a significant impact in the lives of the citizens of South Dakota. The Division of Medical Services is recruiting for a seasoned professional in the area of nursing who is driven by the purpose of the organization: Strong families - South Dakota's Foundation and Our Future. The desired candidate will possess a commitment to service; rigorous attention to detail; the ability to work in an honest, ethical, and responsible manner; and a desire to increase access to care, strengthen continuity of care and care coordination to improve the health of Medicaid-eligible American Indian recipients in South Dakota.
The primary duties associated with this position will include:
- Enhance care coordination for patients referred by Indian Health Service to a continuum of healthcare providers including but not limited to specialty physician and hospital-based services, long term care services and supports, home and community-based services, and psychiatric residential treatment services for youth.
- Request referrals for appropriate patients and services not provided within Indian Health Service (IHS) and collaborate with primary care providers and other appropriate providers within IHS to complete referral processes.
- Review and update patient medical records for referral services and work closely with both IHS and the outside providers to obtain and place medical records in the IHS EHR.
- Develop working relationships and assist with assigned duties within the primary assigned IHS facility which could include, but not limited to, discharge tracking, immunization tracking, STI tracking, quality measures, and health homes. May also assist with similar duties at other assigned IHS locations.
- Partner with Indian Health Service and Tribal Health Programs to track referred patients for admissions to other healthcare facilities and facilitate the return of patients to Indian Health Service and Tribal Health Programs.
- Track referrals for assigned IHS facilities and ensure they are meeting the requirements of the care coordination agreements.
Licenses and Certifications: Licensed to practice as a registered nurse in the State of South Dakota.
The Ideal Candidate Will Have: An associate or bachelor's degree in nursing with at least two years of nursing experience desired.

- plan and organize work to meet schedules and deadlines and respond and adjust to multiple demands and shifting priorities.
- knowledge of cultural, economic, social, physical, and psychological factors that influence customers health care decisions;
- knowledge of industry standards for continuous quality improvement and CQI measurement tools;
- knowledge of desktop computer, including Microsoft Word, Excel, Access, and mainframe applications.
This position is eligible for Veterans' Preference per ARSD 55:10:02:08.
