MDS Coordinator, RN/ LPN

Richland, PA 

Phoebe Ministries is hiring a Part-time MDS Coordinator, RN/ LPN to join our long-term care facility in Richland, PA.

Work Schedule: 24 hours a week/ Monday to Friday, NO WEEKENDS NO HOLIDAYS.

Responsibilities for a MDS Coordinator with Phoebe Richland:

1. Responsible for assessment scheduling and completion of the MDS in accordance with the RAI guidelines and with all practices set forth by the Pennsylvania Department of Health and Federal CMS.
2. Oversees Case Mix process and works closely with therapy and MDS coordinators scheduling UDA’s and assessment.
3. Monitors and completes all required processes in accordance with the Federal Medicare and PA Medicaid Guidelines. All MDS Coordinators will operate in accordance with the practice parameters as set forth in the State Nurse Practice Act according to their licensure.
4. Coordinates and schedules care conference meetings per regulations
5. Review and evaluate QM’s and QRP reports and attend meetings to discuss
6. Participates in triple check process.

Requirements for a MDS Coordinator with Phoebe Richland:

1. The ability to deliver excellent customer service to all residents, families, and coworkers.
2. Graduate of an approved school of nursing and currently licensed in Pennsylvania to practice as a Registered/ Licensed Nurse
3. MDS Coordinator and case mix experience is preferred
