The Community Health Worker (CHW) is responsible for the implementation of referral strategies to proactively identify appropriate patients for Diabetes Self-Management Training (DSMT) that will help patients form good habits and improve overall health and quality of life. In this position, the CHW will have the opportunity to be part of an exciting new multi-disciplinary team that will make a positive impact in healthcare across the continuum of care.
This position focuses on increasing the referrals and connections to outpatient DSMT.
This position will assist the hospital multi-disciplinary team, consisting of the individual’s attending physician, medical specialists, diabetes educator, transition guide nurse, social worker, case manager, hospital nursing staff, family, community agencies and informal support networks, with the communication, coordination and collaboration necessary for the success of the patient after leaving the hospital.
This position requires patient contact via in-hospital visits and post discharge telephone calls.

EXPERIENCE/REQUIREMENTS:
Bachelor’s Degree in Healthcare related field
Preferred home health, community case management or public health experience.
Experience in community systems, entitlements and third party reimbursement preferred.
Bilingual language capability desirable.
Salary Range: Minimum 20.22 per hour - Maximum 33.38 per hour. Compensation will be commensurate with equity and experience for roles of similar scope and responsibility.
In cases where the range is displayed as a $0 amount, salary discussions will occur during candidate screening calls, before any subsequent compensation discussion is held between the candidate and any hiring authority.
