**Job Title: Behavioral Health Care Coordinator**
**About the Role**
We are seeking a dedicated Behavioral Health Care Coordinator to join our dynamic care team! In this role, you will work closely with primary care providers to facilitate care coordination and counseling for patients with mental health needs. **Responsibilities:**
- **Medication Management:** Assist in monitoring patient compliance with medication prescribed by physicians or advanced care providers.
- **Patient Partnerships:** Facilitate collaborative relationships between patients, their personal providers, and family members when appropriate.
- **Psychotherapy and Progress Tracking:** Provide psychotherapy services and track patient progress between provider appointments.
- **Resource Connection:** Inform patients about available community services and resources to support their well-being.
- **Care Plan Coordination:** Review care plans and charts, coordinating with care providers to ensure they are being followed accurately.
**Qualifications:**
- **Education:** Bachelor's Degree in Sociology, Psychology, or a related field.
- **Experience:** Proven experience as a Behavioral Health Specialist or Clinical Social Worker in a clinical setting.
- **Teamwork:** Ability to work effectively as part of a multi-disciplinary team.
- **Regulatory Knowledge:** Understanding of health and patient care regulations.
- **Communication:** Excellent communication skills to engage with patients and colleagues effectively.
- **Ethics:** Strong ethical principles to uphold patient confidentiality and professional standards.
